Johnny Igaz created this song as a tribute to his younger sister Teresa Igaz. Johnny started work on the song on January 13, 2016 and completed on January 17. Teresa, who had struggled for years with schizoaffective disorder, died on December 9, 2015 from an overdose of prescription drugs consumed while she was experiencing a psychotic episode. She was 30 years old.

The title "Milkweed Water Doctor" is a reference to a childhood character Teresa would play. The "doctor" was a quack healer, and whenever she would attempt to treat someone's illness, her arm would snap off, oozing sap and revealing that her whole body was made of milkweed. The sound was inspired by Teresa's love of jungle and drum and bass music when she was attending parties in the early 2000s.
